boil them in water. If they do not open up...discard!

... clean the outside shell of the live clams with a brush under cold water and then put them in pot. Fill the pot to cover the clams with cold water and stir in 1/4 cup of oatmeal. Put in the fridge for 1 hour. The clams will open-up a little and the oatmeal will draw any sand that is on the inside of the shell and some think they will actually eat a bit of the oatmeal and release any sand in their digestive tract. Rinse in cold water. ... and as kay-gee says discard any that do not open after cooking.

... that would be for butter clams and other small clams.
... my favorite way to cook is to place them on an oven rack over an open fire at the beach. As they open the liquid falls into the coals causing them to be lightly smoked. Use a crab pick or small fork. Melt some garlic butter to dip them in. ... gotta go ... getting hungry ...
